What about a character generator to start? One that is fully customizable for sex/gender, since most games adhere to the false binary? You did a great job on your moodtheme by creating a blank head and then applying different features to it (blinking eyes, the pouring rainbow, etc.) that can mix and match. So the base bodies might be tall/average/short, thin/average/chunky, feminine/androgynous/masculine, etc. Then different hairstyles, eye colors, outfits, whatever. Maybe some fey options like wings or horns. Play around with it and see if it inspires a game direction.

Okay, what about a list of sex/gender identities to choose from? And a list of different prompts that would help shape a storyline? The Bingo Card Generator has lots of ideas for characters, settings, plot events, etc. Maybe you could come up with a setting where discovering one's identity is a fun adventure instead of a demolition derby like here.

A game that might be useful for you to read is this one, by an old internet acquaintance: https://aphrobitey.itch.io/this-body-of-mine
A meditation on bodies (and bodily dysphoria) through the lens of mecha and leaving a mech war

I also am a huge fan of Brave Sparrow, which I think is somewhat of a lyric game (where the game exists mostly as a text to be experienced rather than traditionally played). https://buriedwithoutceremony.com/brave-sparrow
It is gender and trauma and learning to break out from hegemony. You are a sparrow trapped in the body of a human, trying to navigate human society and regrow your wings.

Point being there are many interesting lenses to take on the topic through, and if you can figure out the lens and perspective you want to work with, you can make something really resonant and interesting. Both of these games ended up being integral to my own understanding of transness and gender, to the point that I suspect I'll have to write a DW post about it eventually.
